Tirzepatide dosage by administration route

Tirzepatide injectable dosage

Tirzepatide starts at 2.5 mg once weekly for 4 weeks, then increases to 5 mg weekly; further increases are 2.5 mg at intervals of at least 4 weeks. For Mounjaro, the adult maximum is 15 mg weekly and the pediatric maximum is 10 mg weekly. Zepbound maintenance doses are 5 mg, 10 mg, or 15 mg weekly for weight reduction and 10 mg or 15 mg weekly for obstructive sleep apnea. An alternative schedule uses 2.5 mg weekly in weeks 1–4, 5 mg in weeks 5–8, 7.5 mg in weeks 9–12 and 10 mg from week 13. These are escalation steps, not a requirement to increase at every step; they do not replace product-specific maintenance doses up to 15 mg.[1][2][10]

Tirzepatide activates both GIP and GLP-1 receptors. Labeled injectable products may support weight management or type 2 diabetes control in their indicated populations.[1][2][3][4]

Tirzepatide sublingual — compounded 15 mg/ml suspension dosage

A specific compounded sublingual suspension contains 15 mg/mL tirzepatide. Its introductory schedule uses 0.3 mL daily in week 1, 0.4 mL in week 2, 0.5 mL in week 3 and 0.6 mL in week 4, corresponding to 4.5, 6, 7.5 and 9 mg per administration. These formulation-specific directions are not dose equivalents for weekly injections.[11]

Tirzepatide acts at GIP and GLP-1 receptors. A reproducible absorption profile and treatment effect from a sublingual preparation have not been established.[5][6][1][2][3][4]

Frequency and timing

For injections, use the same day each week, at any time and with or without food. A missed injection can be taken within 4 days; after that, skip it and resume the regular weekly schedule. These instructions apply to injections, not the daily compounded sublingual preparation.[1][2][11]

Cycle length and breaks

Injectable: Ongoing. Sublingual — compounded 15 mg/mL suspension: 4-week introductory schedule. Ongoing treatment does not follow a routine on/off cycle.[1][2][11]

Tirzepatide safety and side effects

Approval and product quality

Tirzepatide injection appears in FDA-approved labeling for Zepbound and Mounjaro; indication depends on the specific product label.[1][2]

Side effects and long-term safety

Gastrointestinal symptoms are common, particularly during dose escalation. The prescription label also addresses serious risks and interactions that depend on the treatment context.[1][2]

Route-specific dosing limits

Key cautions are the boxed thyroid C-cell tumor warning, pancreatitis or gallbladder disease, dehydration-related kidney risk, hypoglycemia with insulin or sulfonylureas, oral contraceptive exposure, and unapproved GLP-1/GIP products. Additional schedules attributed to Everything Peptide are reported patterns; citing research does not make these schedules clinically established or approved product instructions.[1][2][9][10]

Common Tirzepatide dosage questions

Can Tirzepatide doses be transferred between routes?

No. Each route and formulation has its own exposure and administration requirements. Use the dose information for the exact presentation rather than copying a tablet, injection or local-product amount to another route.[1][2][3][5][6]

What dosage is listed for Tirzepatide?

Tirzepatide starts at 2.5 mg once weekly for 4 weeks, then increases to 5 mg weekly; further increases are 2.5 mg at intervals of at least 4 weeks.[1][2][10]

Does Tirzepatide have a standard cycle or break interval?

Injectable: Ongoing. Sublingual — compounded 15 mg/mL suspension: 4-week introductory schedule. Ongoing treatment does not follow a routine on/off cycle.[1][2][11]
