SS-31 dosage chart
This summary table compares available dose amounts and schedules by administration route.
| Route | Dose | Frequency & duration |
|---|---|---|
| Injectable | 40 mg daily (Barth syndrome; FORZINITY) | Once daily · Ongoing |
| Ophthalmic | 1 drop of 1% solution per treated eye | Twice daily · 52 weeks |
SS-31 dosage by administration route
SS-31 injectable dosage
For Barth syndrome in patients weighing at least 30 kg, the FORZINITY label gives 40 mg subcutaneously once daily. For adults with severe renal impairment (eGFR below 30 mL/min/1.73 m²) who are not receiving dialysis, the labeled dose is 20 mg once daily. The label does not establish a renal-adjusted regimen for pediatric patients or adults receiving dialysis. An alternative schedule uses a weight-based 0.05–0.25 mg/kg daily SC or IV range for 4–12 weeks. This is separate from the labeled 40 mg subcutaneous FORZINITY regimen for Barth syndrome and must not replace it or supply an IV preparation instruction.[1][10]
Elamipretide associates with cardiolipin in the mitochondrial inner membrane. FORZINITY is a specific approved formulation for Barth syndrome, rather than a general mitochondrial-wellness regimen.[2][3][4]
SS-31 ophthalmic dosage
The investigational ophthalmic schedule uses one drop of 1% elamipretide solution in each treated eye twice daily for 52 weeks. This sterile eye formulation is separate from injectable SS-31 and cannot be prepared from an injection vial.[9][11]
An eye formulation delivers elamipretide locally rather than using the FORZINITY subcutaneous presentation. Its concentration and application schedule require separate formulation information.[9][2][3][4]

SS-31 safety and side effects
Approval and product quality
FORZINITY elamipretide is FDA-approved under accelerated approval for Barth syndrome; mitochondrial wellness, anti-aging, or compounded SS-31 use is outside that label.[1][7][8]
Side effects and long-term safety
Local injection effects and monitored laboratory or skin findings are relevant to FORZINITY tolerability. Barth-syndrome label data do not establish safety for other doses, non-label mitochondrial-wellness use or unsupervised compounded products.[1][7][2]
Route-specific dosing limits
The Forzinity label highlights hypersensitivity, neonatal preservative risk, renal impairment, and the limits of non-label use. Additional schedules attributed to Everything Peptide are reported patterns; citing research does not make these schedules clinically established or approved product instructions.[1][7][2][10]
